MEDICAL QUESTIONNAIRE Please answer the questions on page 1 and complete the section on the top half of page 2 marked STUDENT.

If all of the answers to the questions on page 1 are NO you are able to dive immediately.

If any of the answers are YES you must go to see a doctor before you are able to dive. If such a visit is needed pages 3 - 6 are provided for the doctor's reference / guidance.

If you are under 18 years of age you must sign the forms and have them countersigned by a parent or guardian.
